The Community Mental Health Transformation Framework is a new model of mental health provision, designed on an asset-based view of communities and integrated working across agencies.

Shaping Community Mental Health Together

An initiative being rolled out across the country the Community Mental Health Transformation Framework (CMHTF) is a new model of mental health provision, designed on an asset-based view of communities and integrated working across agencies.

At the core of the framework is the idea that primary care, secondary care, specialist care, social care, housing, the voluntary sector and commissioned providers should work together in a seamless way, with people who use services at the centre of service provision and much more involved in their own care and support.

We would now like to encourage all those in the VCS who have a stake or interest in the wellbeing of those with experience of serious mental ill health to find out more about this initiative and get involved by taking part in a series of forthcoming consultation events.

Email disabilitiescommissioning@gloucestershire.gov.uk to register your interest in taking part.
